Joseph & Mary’s Home

Medical respite care bridges hospital discharge and shelter. Joseph & Mary's Home is a Cleveland program where adults experiencing homelessness and acute medical needs can recover after they are medically ready to leave the hospital but not well enough to heal on the streets or in a traditional shelter. The home provides short-term care for acute medical needs, including nursing care, nourishing meals, safe shelter and transportation to medical appointments. Residents also receive personalized support and connections to community resources. Staff help residents navigate systems to obtain identification and replace essential documents, while supporting their return to self-care and, if they choose, a path toward housing stability. The program serves adults in Northeast Ohio.
