Community Health Partnership

Provider and clinic staff training is one of Community Health Partnership's strategic priorities. The San Jose organization works with member community health centers and clinics to improve access to health care across its region. Its advocacy focuses on affordable, accessible services for diverse communities, including people with low incomes or no health coverage. The partnership also shares information on finding clinics and learning about Medi-Cal, Covered California, and the Primary Care Access Program. Member clinics offer care regardless of insurance status, with payment based on income. Community Health Partnership addresses barriers to care, supports resources for member centers, and works to strengthen continuity of care. It also provides information about health coverage enrollment counselors and clinics in Santa Clara and San Mateo counties.
