Pathways

Since 1985, Pathways has offered hospice support and guidance for terminally ill clients over 18 and their families. Its three-part Circle of Care also includes care navigation, with personalized care plans for older adults and people with chronic illness, and bereavement programs for children, adults, and families. Support is provided at no cost to clients and their families. Programs include CHANGES family grief support, Journey adult grief groups, and school-based bereavement services. Pathways is a community-based nonprofit serving the Greater Lakewood and Long Beach area, helping people navigate illness, care needs, and loss. Its services focus on supporting older adults' independence, improving quality of life for terminally ill clients, and assisting families through grief before and after a death.
