Rehabilitation and skilled nursing care are provided for people recovering after a hospital stay. Each new resident receives an assessment and an assigned care team that develops a tailored plan, monitors progress, coordinates with physicians and family, and plans for discharge. Services include 24-hour nursing, wound care, cardiac care, memory care, hospice, pain management, palliative care, respite stays, and social services. Inpatient mental health programming for adults and seniors includes dual diagnosis care. Dedicated care areas serve residents with transitional rehabilitation, dementia, and cardiac needs. The center also offers access to medical specialists, including physicians, psychiatrists, psychologists, dentists, podiatrists, audiologists, and optometrists. Located on North Ridge Avenue in Chicago, Park View Rehab Center provides post-hospital care and recovery services.
