Old Irving Park Community Clinic

Located in Chicago, Old Irving Park Community Clinic is a free, volunteer-based primary care and behavioral health facility. It serves uninsured working adults, their families, and unemployed adults without access to government assistance or other financial resources. Patients are seen by appointment Monday through Friday from 9 a.m. to 5 p.m.; the clinic is not an immediate or urgent care facility. Services include general primary care, nutritional support, diabetic counseling, donated prescription medications, medication assistance enrollment, routine laboratory work, and referrals for specialty, hospital, diagnostic, and radiology care. Behavioral health services include mental health and substance abuse assessments, evaluation, treatment referrals, case management, and brief counseling. Cardiology, behavioral health therapy, retinal camera scans, and other specialty services are also available.
