HealthCare Choices NY

A community health center. HealthCare Choices NY is a not-for-profit, New York State licensed diagnostic and treatment center and Federally Qualified Health Center. It provides continuous primary care for newborns, children, teens, and adults, with services including adult and pediatric medicine, women's health, preventive care, physicals, illness and injury care, screenings, and vaccinations. Dental care includes exams and teeth cleaning. Behavioral health services include assessments, brief therapy, and medication-assisted treatment. Specialty care includes physical therapy, podiatry, ophthalmology, gastroenterology, and nutrition, with care navigators also available. Same-day and walk-in appointments are offered, along with evening appointments and nurse triage after hours. Services are available to patients with or without insurance, and eligible patients may receive a sliding fee scale.
