St. Paul's PACE San Diego

A managed healthcare plan for seniors. St. Paul's PACE serves adults age 55 and older with chronic medical conditions who are having difficulty living independently at home. Participants can receive primary medical care, medication management, specialty services, physical therapy, home care, social work assistance, and transportation to appointments. Home care needs are assessed by a nurse. Day centers provide social and dietary support, meals, and activities alongside medical appointments. Care is available through local clinics and virtual appointments, with door-to-door transportation for medical visits. Services are offered exclusively to St. Paul's PACE participants. The program operates throughout San Diego County and is part of St. Paul's Senior Services.
