Alegria Collaborative

Care management connects clients' doctors' offices with their homes. Alegria Collaborative provides companion services for people with complex psychiatric and substance addiction conditions, including high-acuity cases. Its work includes safety support, medication management, clinical insight, care coordination, transportation, supportive aides, and family support. The organization works with psychiatrists and residential psychiatric and addiction programs, and supports clients with substance use disorders, behavioral issues, and psychiatric conditions. Care is tailored to individual needs, goals, and management plans. The team also provides coaching and case management, with clinical notes and communication for providers. Alegria Collaborative serves clients from across the world and offers support at different levels of care.
